FORM 5500 E-FILE PORTAL

This portal will allow you to securely review and submit your plan's Form 5500 to the Department of Labor.

This new and enhanced portal will require you establish a new password for 2020. Enter your user name then click on the "Forgot Your Password" link. Follow the instructions to create a new password.

Your user name will be emailed to you once your Form 5500 has been published to the portal.
